ตามคำแนะนำของ American Heart Association ปี 2004 Ideal cuff ควรจะมีขนาดของ bladder length ที่ ร้อยละ 80 และ bladder width อย่างน้อย ร้อยละ 40 ของ arm circumference (length-to-width ratio = 2:1) โดยได้มีคำแนะนำ ดังนี้่ 1.สำหรับ arm circumference 22-26 เซนติเมตร (ซม.) ใช้ cuff สำหรับ small adult : 12 x 22 ซม. 2.สำหรับ arm circumference 27-34 เซนติเมตร (ซม.), ใช้ cuff สำหรับ adult : 16 x 30 ซม. 3.สำหรับ arm circumference 35-44 เซนติเมตร (ซม.), ใช้ cuff สำหรับ large adult : 16 x 36 ซม. 4.สำหรับ arm circumference 45-52 เซนติเมตร (ซม.), ใช้ cuff สำหรับ adult thigh : 16 x 42 ซม. ที่มา Pickering TG และคณะ.
